What is Pathology - What causes granulomas to form?
Granulomas arise in reaction to bacteria and fungi that are difficult to eliminate (e.g., Mycobacterium TB or Histoplasma capsulatum) or chemicals that cause cellmediated hypersensitivity rather than humoral hypersensitivity. Macrophages that arrive at the injury site ingest the noxious substance and become active. Chemokines are released by activated macrophages to attract additional macrophages and lymphocytes. At the same time, macrophages morph into epithelioid cells, which become stationary and prefer to stay in the granulomas rather than travel, thanks to cytokines generated by themselves and lymphocytes. Some epithelioid cells merge into multinucleated giant cells when they are exposed to interferon-g.
