The window for systemic thrombolytics was extended from 3 to 4.5 hours by more recent trials including the European Cooperative Acute Stroke Study (ECASS) III in 2008 and the SITS-ISTR trial, indicating a similar functional benefit and risk of ICH in this extended window. The most crucial thing to keep in mind is that functional outcome and risk of adverse event are time-dependent, and in patients who qualify, thrombolytic therapy should be started as soon as possible.
