Cardiovascular History – Chest Pain
-Approach patient -introduce yourself -take the patient’s name, date of birth and occupation -used SOCRATES when pain is mentioned. -Site -Onset: when does the chest pain occur? 1)during rest / exertion 2)exercise tolerance ( how far you can walk on flat, hill, stairs without discomfort or problem) -character: sharp, crushing, dull, ache -radiation : arm, jaw, back -Associated with : sweating, shortness of breath, nausea and vomiting, loss of consciousness, palpitations -Timing : when is it worst, happens all day ( morning to night, waking from sleep) -easing factors / aggravating factors – resting, GTN spray, influenced by meal,worse in the cold -severity: grade from 1to 10 1-very mild and 10 very severe.
